The Patient-Centered Medical Home (PCMH) is a model that sets a national standard for primary care that emphasize care coordination and ongoing quality improvement,” PCMH recognition offers health centers a structure to pursue systems changes that move them towards delivering higher quality care to patients at lower cost. A majority of HRSA-funded health centers and look-alikes have PCMH program recognition—and that number is growing.
