Community Health Center of the Future: In partnership with the Mid-Atlantic Telehealth Resource Center, NACHC is developing comprehensive Digital Health Innovation trainings based on the Health Center of the Future Model. These trainings provide practical guidance for community health centers to prepare for a sustainable and adaptive future.
Health Center of the Future
The intent of the Health Center of the Future Model is to provide a framework to guide health centers to effectively prepare for a sustainable future.
This framework was collaboratively developed in 2022/2023 by a team of health center experts convened by the Mid-Atlantic Telehealth Resource Center.
Mission: Develop “a cohesive set of a wide range of insights and pragmatic recommendations across a comprehensive set of elements that describe the Health Center of the Future, coalescing the diverse expertise of a multidisciplinary team.”
The Value: Offer Community Health Center Leaders a valuable, pragmatic guide on how to prepare for and move towards transforming their organization into a “Health Center of the Future”

Health Center of the Future Model
At its core, it describes the three key critical aspects of modern health centers that need to embrace a community-focused care model, leverage enhanced partnerships and relationships, and develop and implement multiple evolving business models for revenue generation.
This transition is facilitated and empowered by a suite of enabling & supportive technologies and a set of high-performance capabilities.
When the Health Care Center of the Future is depicted as a car, the Community-Focused Care Model becomes the engine, with the Evolving Business Models and resultant (new) Revenue Sources presenting the fuel, with the car operating and driving in an environment created by Enhanced Partner- and Relationships.
In the driver seat is a leadership team that is leading a high-performing organization (enabled by high-performance capabilities) whereas the enabling and supportive technologies and infrastructure are represented by the car’s systems and chassis/body.
