Overview
The United States is facing a maternal health crisis. According to the Centers for Disease Control and Prevention, Community Health Centers are well positioned to increase access to maternal care. NACHC is working with several partners to improve postpartum care for health center patients by developing partnerships among with Maternal Mortality Review Committees (MMRCs) and Perinatal Quality Collaboratives (PQCs) and health center systems for integrated data-decision making and streamlined approaches to evidence-based practices.

Improving Quality in Pregnancy and Postpartum Care Implementation Guide
To reduce maternal mortality rates, we encourage health center controlled networks (HCCNs) and health center staff to leverage data from electronic clinical data systems for the full continuum of pregnancy and postpartum care. Action steps outlined in this guide are essential for more equitable family planning, prenatal care, transition to primary care, and postpartum care.
