This four-part learning collaborative will combine choosing evidence-based interventions with practical implementation approaches that enhance health equity and prepare health centers for value-based care. These methods have been tried and tested with participating Million Hearts® health center teams. The series will build your capacity to identify and deploy the optimal implementation strategies for your unique patient population and environment.
