On July 20 at 12:30 pm, the National Association of Community Health Centers (NACHC) will be hosting the second of a two-part LinkedIn Live Series. Heart to Heart: Empowering Patients in their Blood Pressure Control, which will feature an interactive and informative discussion highlighting:

  • the critical role Self-Measured Blood Pressure (SMBP) plays in helping patients manage their hypertension
  • best practices for implementing SMBP telemonitoring programs in community health centers and other health care settings
  • overcoming barriers to starting an SMBP telemonitoring program
    how SMBP increases patient engagement and joy in work for care teams
    SMBP program tools and resources
