Addressing SDOH through Collaboration: Lessons from Community Clinic (AR) and The Wright Center (PA)

Overview

Communities are seeking strategies to address the health impacts of social drivers of health (SDOH). The National Association of Community Health Centers (NACHC), with support from the Robert Wood Johnson Foundation, facilitated “Design Sprints” starting in 2021, using human-centered design to incorporate community voices into cross-sector programming. These sessions equipped teams with strategies to improve health equity. Hear from Community Clinic in Bentonville, Arkansas and The Wright Center in Scranton, Pennsylvania on their diverse approaches to engaging communities and enhancing relationships to address SDOH for their patients.

Learning Objectives

  • Apply Human-Centered Design: Learn to use human-centered design principles to address health impacts of structural racism and social drivers of health.
  • Enhance Community Engagement: Discover diverse strategies for engaging community members and gathering perspectives to foster meaningful health interventions.
  • Advance Health Equity: Gain insights into innovative approaches for developing community-driven strategies to improve and sustain health equity through cross-sector collaboration.
