Clinical Care and Quality
The Clinical Care and Quality program at NACHC develops clinical leadership and excellence at our nation’s Community Health Centers. Our main areas of focus are: Clinical Workforce, Care Delivery, Quality Center, and Informatics.
Care team members serving as health center leaders need progressive, evidence-based learning strategies to achieve clinical, operational, administrative, and organizational excellence. The NACHC-EXCEL (Excellence in Clinical Executive Leadership) program and partners offer in-person courses, self-paced learning modules, and one-on-one coaching services to empower clinical providers (physicians, dentists, pharmacists, nurses, physician assistants, etc.) – especially during their first five years in a leadership position. We offer:
- Competency-based self-assessments and resources
- Clinical Leadership Training
- Clinical Leadership Coaching
- Office hours
- Timely webinars on emerging issues with external partners
NACHC improves health equity and health outcomes through impactful and far-reaching special projects including evidence-based, culturally informed, practices – scaled through impactful care delivery pilot projects including:
- Million Hearts ® heart disease and stroke prevention
- Adult Immunization and Vaccine Ambassador programs
- Adolescent Sexual and Reproductive Health (ASRH) guidance for health centers
- Pediatric Weight Management programs
- Prevention efforts for maternal mortality
- Substance/Opioid Use Disorders guidance for health centers
NACHC’s Quality Center develops opportunities and resources grounded in the Value Transformation Framework (VTF) which guides health centers to provide value-based care. “Value-based care” means improvements in health outcomes, patient satisfaction, provider satisfaction, costs, and equity can be achieved (the Quintuple Aim).

Invests, improves, organizes, and empowers health center staff with tools for data governance, data integrity, and data standards. We expand technological efficiencies in the use of patient and care team data by offering:

- Special projects to close care gaps using data, data dashboards, and other informatics tools
