Overview

This four-part learning collaborative will combine choosing evidence-based interventions with practical implementation approaches that enhance health equity and prepare health centers for value-based care. These methods have been tried and tested with participating Million Hearts® health center teams. The series will build your capacity to identify and deploy the optimal implementation strategies for your unique patient population and environment. Join us to achieve your blood pressure and health equity goals.

Learning Objectives

  • Understand the importance of implementation planning to reach blood pressure control quality improvement goals and do so in an equitable manner.
  • Identify the most impactful implementation strategies for your local context.
  • Create an implementation plan to improve blood pressure outcomes and health equity in your organization
